Still getting the "invalid" message and zero results.

I always fill out the permit form if there are any available. The USFS uses this data for trail maintenance and other issues.

Another vote for always filling one out if they are there. If you want to know how these get used you should talk to Dr. Bo Shelby at OSU, had a class with him called Recreation Planning and Management. This was a very interesting eye opener to see how the differing agencies manage recreation. Another good course was Wilderness Management. http://fes.forestry.oregonstate.edu/faculty/shelby-bo

Always fill them out. I actually had one checked for the first time coming down from Camp Lake a couple weeks ago.

I fill it out, but have stopped putting in my address. I can't imagine why they need to know that. I just put Portland.

If I get on the news as a lost hiker, it's not like having my address on the form will make the difference!

I've always been under the impression that one way the Forest Service knows who is 'up there' is by collecting the forms. Maybe that was the original intention?

We've filled them out on every trip, when there are forms in the box.
